Effective January 1, 2010, Anthem Blue Cross and Blue Shield (Anthem) will implement prior authorization requirements for outpatient physical and occupational therapy services for certain health benefit plans in Connecticut.  Orthonet will perform a utilization review or consultation review, as appropriate.  The initial evaluation does not require utilization or consultation review.  In addition, providers are not required to obtain prior authorization for members already in a course of treatment for services that are scheduled prior to January 1, 2010 for dates of service between January 1 and January 31, 2010.
